Athlete Forms:
Student athletes who are interested in joining the track team will need to have all required paperwork turned in by the first day of practice to any of the coaches or to the High School Athletic Office.
ALL REQUIREMENTS MUST BE MET:
1 - All sport participants must have a Participation Form (only need one per school year) filled out and on file in the athletic office before they can participate.
2 - Updated physical information (page 2 of the Participation Form above). Physical must be after April 1 of previous year.
3 - A $100.00 participation fee is due for each sport, with a family cap of $400. This can be paid online or checks can be made out to MHS.
4 - Read and sign the Concussion Form only need one per school year - Reference the Parent Concussion and Athlete Concussion information flyers.
